A prolapsed hemorrhoid is an internal hemorrhoid that protrudes out the anus. It will usually appear as a painless (or nearly painless), moist red mass of tissue covered with rectal mucose. A prolapsed internal hemorrhoid can become thrombosed (having a blood clot), and can be very painful.

Sometimes, a prolapsed hemorrhoid will only protrude upon bowel movement, and will spontaneously retract itself. Other times, the prolapsed tissue must be manually re-inserted into the rectum. The most severe kind of prolapsed hemorrhoid is usually strangulated or thrombosed and can’t be manually replaced because it will just keep coming out.

A prolapsed hemorrhoid is a second, third or fourth degree hemorrhoid (first degree hemorrhoids being those that do not protrude outside of the anal canal). A second degree is a prolapsed hemorrhoid that spontaneously retracts. A third degree is a prolapsed hemorrhoid that can be manually re-inserted. Symptoms of hemorrhoids range from a mild itch or burn, then to slight or moderate bleeding, and all the way to large, painful masses of protruding tissue (prolapsed hemorrhoids). While these protrusions of tissue are scary, gross and very uncomfortable, they are usually not painful. Usually, only thrombosed prolapsed hemorrhoids cause pain. Some of the most common symptoms of hemorrhoids are rectal itching, spongy lumps around the anus, blood in the stools or found on the toilet tissue, not being able to finish a bowel movement, and/or soiled undies. Pain upon defecation is usually not a symptom of hemorrhoids, and should immediately be examined by a physician.

External hemorrhoids occur outside the rectum, around the anus. Basically, all swellings or protruding masses at or near the anal opening are considered to be external hemorrhoids (except in the case of prolapsed internal hemorrhoids, which originate internally but can protrude externally as they worsen).
