Thrombosed hemorrhoids are hemorrhoids that have developed a clot on the inside (a ‘thrombus’). These clots form in the veins of the rectum or anus, just under the skin. External thrombosed hemorrhoids can be seen with the naked eye, and/or felt during physical examination – sometimes they are soft, like raisins; other times, they are hard, like frozen peas. Thrombosed hemorrhoids, because they involve the strangulation or plugging of a vein just under the skin, usually turn slightly purple or blue; the outer skin can also redden, if the hemorrhoid is irritated and becomes inflamed. Though quite often painful, thrombosed hemorrhoids do not usually require surgery. They can be effectively treated with warm baths, creams and a change in diet (to include more fiber, thus loosening or softening the stools). In the case of more serious thrombosed hemorrhoids, when surgery is indicated, the clot can be drained or excised, or the entire section of affected vein can be removed.

A thrombosed external hemorrhoid is a hemorrhoid in which blood has pooled and formed a clot (thrombus). A thrombosed external hemorrhoid can be diagnosed by visual and/or rectal exams. This type of hemorrhoid occurs outside the rectum, around the anal region. It will usually appear as bulging, purple or bluish skin-covered veins, or can be reddish when inflamed. To the touch, a thrombosed external hemorrhoid usually feels like a small, hard lump, roughly the size of a pea. Occasionally, this type of lump is referred to as a “skin tag”, and sometimes feels like a small mass of extra skin. With this type of hemorrhoid, most sufferers experience some degree of pain, often quite a lot. A thrombosed external hemorrhoid can cause swelling, itching and/or pain, but it will almost never bleed. The usual treatment for a thrombosed external hemorrhoid is drainage or removal (hemorrhoidectomy) of the clot or the entire hemorrhoid.
